Dilantin

CLASS and MECHANISM of Action

Anticonvulsant drugs can elevate the seizure threshold and/or limit the spread of seizure discharge. Generic "Dilantin" exerts its anticonvulsant effect mainly by limiting the spread of seizure activity and reducing seizure propagation, unlike phenobarbital and carbamazepine, which elevate the seizure threshold. Because Generic "Dilantin" does not elevate the seizure threshold, it is less effective against drug-induced or electroconvulsive-induced seizures. Generic "Dilantin" 's anticonvulsant effects are mediated through effects on sodium channels on the neuronal cell membrane. Generic "Dilantin" exerts its anticonvulsant effects with less CNS sedation than does phenobarbital. In toxic concentrations, Generic "Dilantin" is excitatory and can induce seizures. Generic "Dilantin" is also a weak antiarrhythmic. Antiarrhythmic actions are also mediated through effects on sodium channels, in this case, in Purkinje fibers.

Active Ingredients: Phenytoin sodium.
Inactive Ingredients: Sucrose, lactose, starch-wheat, purified talc, magnesium stearate, titanium dioxide, erythrosine, carbon black and gelatin.

What is Generic "Dilantin" PRESCRIBED For?

Generic "Dilantin" is indicated for the control of generalized tonic-clonic (grand mal) and complex partial (psychomotor, temporal lobe) seizures and prevention and treatment of seizures occurring during or following neurosurgery.

How Should I Take Generic "Dilantin"?

The usual adult dose is 300 mg to 400 mg daily taken by mouth in divided doses or a single dose. An increase to 600 mg taken in divided doses may be needed in some cases. The children's dose is based on age and body weight. The level of Generic "Dilantin" in your blood can be checked through laboratory testing. This test helps your doctor determine the dose of Generic "Dilantin" that best suits you.
Generic "Dilantin" may be taken with or without food. Do not use capsules that are discoloured.
Many things can affect the dose of medication that a person needs, such as body weight, other medical conditions, and other medications. If your doctor has recommended a dose different from the ones listed here, do not change the way that you are taking the medication without consulting your doctor.
It is very important that this medication be taken exactly as prescribed by your doctor.

Keep out of the reach of children in a container that small children cannot open.
Store at room temperature between 15 and 30 degrees C (59 and 86 degrees F). Keep container tightly closed. Throw away any unused medicine after the expiration date.

What are the CONTRAINDICATIONS for taking Generic "Dilantin"?

Generic "Dilantin" should not be taken by anyone who is allergic to phenytoin sodium or other medications in the class of medications called hydantoins.

What Happens if I MISS A DOSE?

Try not to miss a scheduled dosage, especially if you are taking Generic "Dilantin" extended-release capsules just once per day. If you miss a dose, take it as soon as you can. If it is less than four hours to your next dose, take only that dose. If you only take a dose once a day and do not remember until the next day, skip the missed dose and resume your normal schedule. Do not take double or extra doses.

What happens if I OVERDOSE?

An overdose of Generic "Dilantin" can be fatal. If you suspect an overdose, seek medical attention immediately. Symptoms of Generic "Dilantin" overdose may include: Coma, difficulty in pronouncing words correctly, involuntary eye movement, lack of muscle coordination, low blood pressure, nausea, sluggishness, slurred speech, tremors, vomiting

What are the possible SIDE EFFECTS of Generic "Dilantin"?

Side effects that you should report to your prescriber or health care professional as soon as possible:
  • chest pain or tightness; fast or irregular heartbeat (palpitations)
  • confusion, nervousness, hostility, or other behavioral changes (especially in children or elderly patients)
  • dark yellow or brown urine
  • difficulty breathing, wheezing or shortness of breath
  • double vision or uncontrollable and rapid eye movement
  • fainting spells or lightheadedness
  • fever, sore throat
  • headache
  • loss of seizure control
  • mouth ulcers
  • poor control of body movements or difficulty walking
  • redness, blistering, peeling or loosening of the skin, including inside the mouth
  • sexual problems (painful erections, loss of sexual desire)
  • skin rash, itching
  • stomach pain
  • swollen or painful glands
  • unusual bleeding or bruising, pinpoint red spots on skin
  • unusual tiredness or weakness

  • clumsiness or unsteadiness
  • constipation
  • difficulty sleeping
  • dizziness or drowsiness
  • excessive hair growth on the face or body
  • loss of appetite
  • nausea
  • stomach upset
  • unusual growth of gum tissue

What other drugs can INTERACT with Generic "Dilantin"?

There are many drugs which may increase or decrease Generic "Dilantin" levels or which Generic "Dilantin" may affect. Serum level determinations for Generic "Dilantin" are especially helpful when possible drug interactions are suspected. The most commonly occurring drug interactions are listed below:
  • Drugs which may increase Generic "Dilantin" serum levels include: acute alcohol intake, amiodarone, chloramphenicol, chlordiazepoxide, cimetidine, diazepam, dicumarol, disulfiram, estrogens, ethosuximide, fluoxetine, H2-antagonists, halothane, isoniazid, methylphenidate, phenothiazines, phenylbutazone, salicylates, succinimides, sulfonamides, ticlopidine, tolbutamide, trazodone.
  • Drugs which may decrease Generic "Dilantin" levels include: carbamazepine, chronic alcohol abuse, reserpine, and sucralfate. Moban® brand of molindone hydrochloride contains calcium ions which interfere with the absorption of Generic "Dilantin". Ingestion times of Generic "Dilantin" and antacid preparations containing calcium should be staggered in patients with low serum Generic "Dilantin" levels to prevent absorption problems.
  • Drugs which may either increase or decrease Generic "Dilantin" serum levels include: phenobarbital, sodium valproate, and valproic acid. Similarly, the effect of phenytoin on phenobarbital, valproic acid, and sodium valproate serum levels is unpredictable.
  • Although not a true drug interaction, tricyclic antidepressants may precipitate seizures in susceptible patients and Generic "Dilantin" dosage may need to be adjusted.
  • Drugs whose efficacy is impaired by Generic "Dilantin" include: corticosteroids, coumarin anticoagulants, digitoxin, doxycycline, estrogens, furosemide, oral contraceptives, paroxetine, quinidine, rifampin, theophylline, vitamin D.

Additional Information

If you are pregnant or plan to become pregnant, inform your doctor immediately. Because of the possibility of birth defects with antiepileptic drugs such as Generic "Dilantin", you may need to discontinue the drug. Do not, however, stop taking it without first consulting your doctor. Generic "Dilantin" appears in breast milk; breastfeeding is not recommended during treatment with this drug.
