Nitroglycerin

CLASS and MECHANISM of Action

Generic "Nitroglycerin" - an organic nitrate with vasodilator activity. Generic "Nitroglycerin" is converted into nitric oxide (NO) in smooth muscle and activates guanylyl cyclase, thereby increasing cGMP concentration, and resulting in smooth muscle relaxation. Dilatation of the veins results in decreased venous return to the heart, thereby decreasing left ventricular volume (reduced preload) and decreasing myocardial oxygen requirements. Arteriolar relaxation reduces arteriolar resistance (reduced afterload), thereby decreasing myocardial oxygen demands. In addition, Generic "Nitroglycerin" causes coronary artery dilatation, thereby improving myocardial blood distribution.

Active Ingredients: Dextrose, nitroglycerin
Inactive Ingredients: Polyester film, silicone and acrylic adhesive with a cross-linking agent.

What is Generic "Nitroglycerin" PRESCRIBED For?

    Generic "Nitroglycerin" is used as a heart medication, mainly in the following cases:
  • Prophylaxis and treatment of angina and left ventricular failure.
  • Control of hypertension during cardiac surgery.
  • Congestive cardiac failure unresponsive to usual therapy.
The principal action of Generic "Nitroglycerin" is vasodilation—widening of the blood vessels.
    Generic "Nitroglycerin" will dilate veins more than arteries, decreasing cardiac preload and leading to the following therapeutic effects during episodes of angina pectoris:
  • subsiding of chest pain
  • decrease of blood pressure
  • increase of heart rate
  • orthostatic hypotension
These effects arise because Generic "Nitroglycerin" is converted to nitric oxide in the body (by a mechanism that is not completely understood), and nitric oxide is a natural vasodilator.

How Should I Take Generic "Nitroglycerin"?

Generic "Nitroglycerin" shoukld be taken by mouth with or without food. The dosage sould be indicated by your doctor. However, usually it is taken:
Buccal:
1 to 5 mg three times a day (angina).
5 to 10 mg three times a day (congestive cardiac failure). Oral:
2.5 to 10 mg as sustained release tablets, two to three times daily.

No specific recommended dose for children.

Generic "Nitroglycerin" tablets are unstable unless they are stored under air-tight conditions and protected from light.

What are the CONTRAINDICATIONS for taking Generic "Nitroglycerin"?

    Generic "Nitroglycerin" is contraindicated in the following cases:
  • Cerebral haemorrhage.
  • Head injury.
  • Severe anaemia.
  • Hypotensive conditions.
If you take any medicines that have nitrates in them (like nitroglycerin for chest pain)—every day or even once in a while—you should NOT take VIAGRA.

What Happens if I MISS A DOSE?

If several hours have passed and it is almost time for the next dose, skip the missed dose and take only the next regularly scheduled dose. Do not take a double dose of the medication to catch up.

What happens if I OVERDOSE?

Methaemoglobinaemia may occur in patients following an overdose. Patients with severe acute Generic "Nitroglycerin" overdose should be admitted to an intensive care unit. Keep patient in head low position.

What are the possible SIDE EFFECTS of Generic "Nitroglycerin"?

Hypotension, reflex tachycardia and palpitations may occur. Postural hypotension and syncope is seen, especially in elderly patients. Rarely severe bradycardia has been reported. Throbbing headache is quite common. This symptom is likely to recede as tolerance develops. Peripheral oedema is also frequently seen.

What other drugs can INTERACT with Generic "Nitroglycerin"?

Alcohol enhances the effects of Generic "Nitroglycerin".

Undue dizziness and faint feeling may occur when sublingual nitrates are taken with beta-adrenoceptor blocking drugs.

Delayed dissolution of Generic "Nitroglycerin" tablets in patients with dry mouths has been reported in a patient taking imipramine and in another patient treated with atropine.

Additional Information

Treatment with Generic "Nitroglycerin"may be associated with lightheadedness on standing, especially just after rising from a recumbent or seated position. This effect may be more frequent in patients who have also consumed alcohol.
