Sildenafil CitrateCLASS and MECHANISM of Action Sildenafil citrate is a drug used to treat male erectile dysfunction (impotence) and pulmonary arterial hypertension.. Part of the physiological process of erection involves the parasympathetic nervous system causing the release of nitric oxide (NO) in the corpus cavernosum of the penis. NO binds to the receptors of the enzyme guanylate cyclase which results in increased levels of cyclic guanosine monophosphate (cGMP), leading to smooth muscle relaxation (vasodilation) in the corpus cavernosum, resulting in increased inflow of blood and an erection.
Sildenafil citrate is a potent and selective inhibitor of cGMP specific phosphodiesterase type 5 (PDE5) which is responsible for degradation of cGMP in the corpus cavernosum. The molecular structure of sildenafil is similar to that of cGMP and acts as a competitive binding agent of PDE5 in the corpus cavernosum, resulting in more cGMP and better erections. Without sexual stimulation, and therefore lack of activation of the NO/cGMP system, Sildenafil citrate should not cause an erection. Sildenafil citrate is metabolised by hepatic enzymes and excreted by both the liver and kidneys. How Should I Take Sildenafil citrate? For most patients, the recommended dose is 50 mg taken, as needed, approximately 1 hour before sexual activity. However, Sildenafil citrate may be taken anywhere from 4 hours to 0.5 hour before sexual activity. If taken with a high-fat meal, there may be a delay in absorption of Sildenafil citrate and the peak effect might be reduced slightly as the plasma concentration will be lowered. Based on effectiveness and toleration, the dose may be increased to a maximum recommended dose of 100 mg or decreased to 25 mg. The maximum recommended dosing frequency is once per day. Sildenafil citrate is effective for 4 hours.

What happens if I OVERDOSE? In case of accidental overdose, call your doctor right away.

What other drugs can INTERACT with Sildenafil citrate? Some medicines can change the way Sildenafil citrate works. Tell your doctor about any medicines you are taking. Do not start or stop taking any medicines before checking with your doctor or pharmacist, this includes prescription and nonprescription medicines or remedies.
If you are taking medicines called alpha-blockers for the treatment of high blood pressure or prostate problems, your blood pressure could suddenly drop. You could get dizzy or faint. If you are taking a protease inhibitor, your dose may be adjusted. Sildenafil citrate should not be used with any other medical treatments that cause erections. Sildenafil citrate comes in different doses (25 mg, 50 mg and 100 mg). If you do not get the results you expect, talk with your doctor. If you are taking protease inhibitors, such as for the treatment of HIV, your doctor may recommend a 25 mg dose and may limit you to a maximum single dose of 25 mg of Sildenafil citrate in a 48 hour period.
